Under the supervision of the Center Manager, the Brandman Centers for Senior Care (BCSC) RN Clinic Supervisor will supervise, coordinate, evaluate and assist in the delivery of clinic nursing services in accordance with BCSC, state and federal standards for providing direct and indirect nursing care to BCSC participants. Works in close coordination with the Primary Care Physician to manage ongoing care and complex clinical situations and support the operation of the clinic.
Administrative Functions
- Assumes the administrative responsibility and accountability for the assigned duties of the Clinic Nurse Supervisor. Coordinates nursing staff to assure that the BCSC standard of nursing care is provided at all times.
- Oversees the hiring and training of all positions in the clinic.
- Completes all of the regulation-required auditing and reporting.
- Provides assistance to all positions in the Clinic, including taking administrative calls.
- Participates in conducting the initial and quarterly assessment of each participant’s health and social status; identifies overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results.
- In coordination with the BCSC nursing staff and the Interdisciplinary Team, develops a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective.
- Participates in providing care that conforms to accepted practice standards and age-related characteristics and needs of participants.
- Assists nursing staff in providing direct care that includes, but is not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician.
- Actively participates in daily morning and Intake and Assessment meetings to update other Interdisciplinary Team members on changes in participants’ condition.
- Supervises and assists nursing staff as required in recording participant assessments, reassessments and care delivered in the electronic medical record.
- Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions.
- Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.
- Ensures that all tests, lab work and diagnostic studies ordered by the Primary Care Physician have been carried out.
- Coordinates all medication for BCSC, including the stocking of non-prescription medications, the ordering of prescription medications, and the administering of medication in the PACE Center.
- Collaborates with the Primary Care Physician to assure that protocols are in place and standing orders are carried out.
- Keeps abreast of current federal and state regulations, as well as professional standards.
- Observe and evaluate nursing staff in the prompt and effective performance of their duties.
- Assists the Center Manager in creating an atmosphere of warmth, personal interest and positive emphasis.
- Prior to the use of physical or chemical restraints, conducts an assessment and documents in the medical record that participant restraint alternatives have been attempted.
- Supports the Infection and Exposure Control function in coordination with the Quality Assurance Coordinator.
- Actively participates in the Quality Assessment and Performance Improvement (QAPI) process.
- Recognizes inappropriate participant care management or service delivery by other Interdisciplinary Team members and addresses directly.
- Identifies emergency situations and takes appropriate action.
- Understands and demonstrates respect for participant rights.
- Coordinates with the Pharmacist and Dentist to support participant care.
- Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions.
- Consistently meets or exceeds BCSC targets for productivity and customer service targets.
- Continually seeks better ways for delivering services and communicating with participants.
- Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality and independence.
- Understands the importance of community involvement and participates as appropriate.
- Provides necessary documentation for ER and urgent care visits, hospitalizations, transfers to skilled nursing facilities, and discharges from the hospital.
Education and Experience
- Degree and Diplomas: Graduate of a school of professional nursing, BS in Nursing preferred.
- Certificates, Licenses: RN currently licensed by the California Board of Registered Nursing. Valid California driver’s license. Verification of completion of CPR (BLS) and first aid training required.
- Experience: Three years nursing experience with at least one year caring for the frail or elderly. A Public Health Nursing background and/or experience with PACE is preferred. At least one year supervisory experience in health care.
- Skills/Aptitudes: Nursing knowledge and skills necessary to treat participants and manage complex clinical situations. Ability to provide care for the frail elderly. Good organizational and supervisory skills.
