Work as part of a multi-disciplinary team, assisting patients and caregivers.
Responsibilities:
- Provide nursing care for physician practice patients following established standards and practices in accordance with the South Carolina Nurse Practice Act.
- Responsible for staging patients in preparation to see providers.
- Update all patient medications in the medication history at each visit.
- Perform patient related tasks as directed by practice providers.
- Ability to draw blood for ordered labs - Phlebotomy Skills.
- Facilitate the timely communication of clinical advice to patients electronically or by phone as directed by practice providers.
- Ensure all needed medical records are available to care team and providers prior to patient's office visit.
- Coordinate with other team members to provide exceptional patient service.
- The LPN should be proficient with the electronic medical record after offered training (EPIC).
- Gather and interpret appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation.
- Document accurately, timely and according to departmental policy.
- Sitting, standing, and walking required throughout the day.
