Description
This position is an integral part of the coordinated care the Health & Wellness Department at Haymarket Center provides to individuals at risk of contracting HIV. This role will assist in increasing knowledge and awareness around HIV and PrEP, and ensuring access to testing, medical care and recovery support services. This role will interact with clients, medical providers, case managers, counselors and other members of the treatment team and is responsible for documentation of interactions and the completion of necessary Release of Information and other documents. Contact with clients will occur both in the treatment setting and in the community.
ESSENTIAL FUNCTIONS:
- Provide education on HIV and STI transmission, prevention and care to groups and individuals.
- Assist in creating access to and providing HIV and STI testing, ensuring test results are delivered.
- Assess client needs and create action plan with client to facilitate linkage to and retention in PrEP services and medical care.
- Complete necessary Release of Information and documentation authorizing of sharing information between Haymarket Center and other service providers.
- Maintain at least monthly contact with assigned caseload of clients.
- Consistently document all contacts with client and about client care in Electronic Health Record in a timely manner.
- Contribute to program development with feedback and sharing of best practices.
- Provide client with health education and coaching to manage medical issues.
- Maintain knowledge on current PrEP and HIV and STI prevention, risk reduction information and treatment options and other subject matters related to job role with approval of supervisor.
- Work with other Haymarket Center staff to ensure client participation in appropriate recovery support services.
- Complete all necessary documentation and reports in a timely basis.
- Participate, as assigned, in Health & Wellness Department activities to improve health education services including development of materials, conducting presentations or supporting other team members in such efforts.
- Attend all staff meetings and scheduled supervisions.
- Other duties as assigned.
Requirements
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
EDUCATION REQUIRED
- High school diploma/GED/or equivalent of 2 years work experience in social service setting required.
- Bachelors in social services or health related field preferred.
EXPERIENCE AND SKILLS REQUIRED:
- Knowledge of HIV, STIs, PrEP and Hepatitis C highly preferred.
- Experience working in health care and navigating health insurance is highly beneficial.
- Experience with Electronic Health Records preferred.
- At least 1 year previous experience working with substance users and knowledge around substance use and abuse required.
- Phlebotomy training preferred.
- Bilingual (English/Spanish) preferred
- Valid Illinois Driver’s License and eligibility for agency insurance coverage is preferred.
- Understanding of HIPPAA, 42CFR, AIDS Confidentiality Act.
- Previous case management experience beneficial.
- Previous counseling and testing experience beneficial.
