How you make a difference
The Registered Nurse Case Manager (CM) coordinates the organization and utilization of resources and healthcare services for persons served. They arrange and oversee medical transportation for persons served and use the nursing process to provide optimal quality and continuity of care. Additionally, the CM integrates utilization review and discharge planning to ensure ongoing provision of high-quality nursing care.
Key Responsibilities
- The Registered Nurse Case Manager (CM) organizes and coordinates healthcare services, evaluates care plans, and provides advice to the nursing staff regarding disease management and community resources.
- The CM monitors clinical care and hospital stays for appropriateness, evaluates nursing outcomes, and participates in quality improvement efforts.
- The CM performs direct nursing care, as needed, and compiles and prepares reports as assigned.
- The CM coaches, counsels, and evaluates clinical staff, implements safety and performance improvement standards, and participates in the development of departmental policies.
- The CM practices principles of risk management, safety, and infection control and intervenes directly in situations that are immediately harmful to persons served.
Qualifications & Requirements
Education
- Graduate of an accredited school of nursing. BSN preferred.
- Psychiatric Certified Preferred.
Experience
- Minimum of (2) two years’ experience as a Nursing Supervisor, Charge Nurse or other management level position in correctional, psychiatric and/or medical surgical nursing.
- Must be able to pass and maintain clearance for a Public Trust Position.
Licenses/Certifications
- Must have a current, unrestricted license to practice as a Registered Nurse (RN) in the state of practice or possess a multi-state compact license recognized by the state of practice professional nursing and maintain a current copy of license on file at the facility at all times.
- Maintains a current BLS certification.
