Position Summary
Rosalind Franklin University Health Clinic's Community Care Connection (CCC) Mobile Screening and Education unit, and the Rosalind Franklin University Health Clinics (RFUHC), welcome applications for a part-time Family Practice Nurse Practitioner/Physician Assistant. The CCC offers health resources at no cost to Lake County residents with a target population of uninsured or underinsured community members. The Family Nurse Practitioner/Physician Assistant will work in our Community Care Connection Mobile Screening and Education unit within Lake County, IL. Hours will vary weekly to provide care at one of our regularly scheduled sites in Lake County and/or guest sites.
The CCC's primary goal is to provide competent, culturally sensitive, respectful, compassionate care while empowering community members to take action to maintain or improve their health. The team promotes the importance of preventative care, disease understanding, and compliance with care plans. Chronic disease management is not done through the mobile clinic; however, the NP/PA will be responsible for connecting community members to the appropriate clinical resource for ongoing healthcare needs. Point of care screening tests and education related to cholesterol, blood pressure, blood sugar, and A1c testing are offered. Management of minor acute illness is also performed from the mobile clinic. Point of care testing for influenza, Covid, strep throat, urinary tract infections, and pregnancy is offered. Childhood vaccinations are offered via the state Vaccine for Children program to Medicaid insured or uninsured children. To support our goal, education workshops are performed in partnership with local community organizations.
This position will require the health care provider to travel to Lake County site locations with the mobile clinic, Community Care Connection (CCC), throughout the year - approximately three to five days per week. The CCC has a target population of community members that are uninsured or underinsured with the goal of empowering community members to take action in their health. The CCC position may require one or two evening shifts between 5-8pm and two weekend shifts per month.

Essential Duties & Responsibilities
- Provide educational presentations to community groups.
- Promote preventative care to all community members.
- Provide and administer health screenings aboard the mobile clinic, in accordance with professional standards and practices, as well as the Health Clinic's policies and procedures.
- Observe, record, and report on a patient's condition and follow-up plan in the EMR, Athena.
- Perform point of care testing and education for cholesterol screening, blood glucose, A1C, rapid influenza, rapid Covid, urine pregnancy, urinary tract infection testing.
- Assess and treat minor sick visit complaints.
- Participate and provide guidance in clinical services available to community members.
- Oversee students and clinical support team participating in screenings and acute care.
- Maintain the Community Care Connection mobile clinic with the necessary supplies and materials. Ensure cleanliness of the room. Prepare lists of medical supplies needed. Prepare equipment, procedures, screenings, and testing of patients.
- Provide patient/family education regarding procedures, medications, lab results, nutrition, and maintenance of health and wellness. Provide follow-up in regard to patient screenings and outcomes.
- Screen patients for appropriate medical history/information.
- Maintain/review patient records, charts, and other pertinent information and post tests and examination results to the patient's chart.
- Utilize community health experience by working with the uninsured, underinsured, and underserved, and provide referrals to patients when appropriate.
- Work with community boards, resource centers, and other community leaders to provide access to healthcare.
- Vaccination clinics and education. Includes childhood vaccinations offered via Vaccine for Children and follow state-mandated guidelines and documentation requirements.
- Educate and provide community members with Narcan.
- Other duties as assigned.
Conditions of Employment

Required Education & Experience
- Physician Assistant (PA) or Nurse Practitioner degree from an accredited school with 1-3 years of experience in a clinical environment and community health assessment and/or educational training.
- Current Illinois licensure.
- Ability to bill Medicare and Medicaid.
- BLS Certification.

- Demonstrated skill in identifying problems and recommending solutions (i.e. critical thinking).
- Demonstrated skill in preparing/maintaining records, writing reports and responding to correspondence via email.
- Demonstrated skill to multi-task and adapt to changes in patient volumes and acuity.
- Ability to maintain quality control standards.
- Ability to react calmly and effectively in emergency situations.
- Ability to communicate effectively and accurately verbally and in writing.
- Ability to apply continuous quality improvement standards to the work process.
- Knowledge of Google applications.
- Legal ability to prescribe prescription pharmaceuticals, narcotic prescriptive authority not required.
- Legal ability to bill Medicare and Medicaid.
Preferred Qualifications
- Demonstrated use of an Electronic Health Record system - Athena.
- English/Spanish speaking.
