Nurse Practitioner/Physician Assistant Primary Care
Job Summary
The Nurse Practitioner/Physician Assistant Primary Care ALF will provide primary care services to patients in an assisted living facility. The successful candidate will work collaboratively with the facility's medical director and other healthcare professionals to provide high-quality care to patients.
Responsibilities
- Conduct initial and ongoing assessments of patients' health status
- Develop and implement care plans based on patients' needs and goals
- Prescribe medications and treatments as appropriate
- Order and interpret diagnostic tests
- Provide patient education and counseling on health promotion and disease prevention
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care
- Maintain accurate and up-to-date medical records
- Participate in quality improvement initiatives
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
- Master's degree in Nursing or Physician Assistant Studies
- Certification as a Nurse Practitioner or Physician Assistant
- Experience in primary care, preferably in an assisted living or long-term care setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
