The Facility Rehab Manager is responsible for overseeing the day-to-day operations of the rehabilitation department at Lawton Post Acute & Rehab. This includes managing a multidisciplinary team of therapists (PT, OT, and ST), and assistants (PTA and COTA), ensuring the delivery of high-quality rehabilitative care to patients, and coordinating with other departments to promote a seamless patient care experience. The Rehab Manager also plays a critical role in optimizing department performance, maintaining compliance with regulatory standards, and ensuring that the department meets its financial and operational goals.
Key Responsibilities:
- Leadership & Supervision:
- Oversee the rehabilitation department, including physical therapists, occupational therapists, speech-language pathologists, and support staff.
- Provide guidance, mentorship, and performance evaluations for rehab staff.
- Foster a collaborative and positive team environment.
- Patient Care Coordination:
- Ensure that all patients receive individualized care plans in accordance with physician orders and regulatory guidelines.
- Collaborate with the nursing and medical teams to ensure continuity of care.
- Monitor patient progress and adjust therapy plans as necessary.
- Compliance & Quality Assurance:
- Maintain compliance with federal, state, and local regulations, including CMS guidelines.
- Ensure accurate and timely documentation of patient care and progress in accordance with facility policies.
- Conduct regular audits to maintain high standards of care and safety.
- Operational Management:
- Develop and monitor the department’s budget, including staffing, equipment, and supply needs.
- Ensure proper utilization of resources and control department costs.
- Work with the leadership team to develop and implement strategies for improving departmental efficiency and patient outcomes.
- Communication & Collaboration:
- Act as the liaison between rehab services, administration, and other departments within the facility.
- Participate in interdisciplinary meetings and provide input on patient care and department needs.
- Communicate with patients, families, and caregivers to ensure they are informed about the rehab process and progress.
- Staff Training & Development:
- Identify training needs and provide continuing education opportunities for rehab staff.
- Stay current with best practices in rehabilitation therapy and implement new methodologies to enhance patient care.
Qualifications:
- Current state licensure as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
- Would be willing to consider the right PTA or COTA.
- At least 3-5 years of clinical experience, with some management or leadership experience preferred.
- Strong knowledge of CMS guidelines, state and federal regulations, and skilled nursing facility practices.
- Excellent organizational, communication, and leadership skills.
- Ability to work collaboratively with a multidisciplinary team.
- Proficient in electronic medical records (PointClickCare) systems and Microsoft Office Suite.

Pay: $68,299.00 - $98,000.00 per year
