Charge Nurse (Senior Suites Basic Care Facility)
Part-time (24 hours/week). Day shift with occasional night, evening, weekend, and holiday shifts as needed.

The charge nurse is a critical thinker, team leader, and team player. The charge nurse is a positive role model and is able to share knowledge freely. Requires knowledge and oversight of basic care licensing regulations. The charge nurse is accountable for delivering care to the resident utilizing the nursing process. Responsible for admission of new residents; overseeing and implementing the resident care plans; establishing communication with residents, families, physicians, other staff; and functioning as a resident advocate.
The charge nurse will be responsible for assisting the Director with the day-to-day operations of the facility including ensuring that staff are trained to perform the duties of their job through orientation and ongoing education; supervision of staff; policy review and implementation; scheduling; and keeping the facility in compliance with federal and state regulations. The charge nurse will assist with overseeing the infection prevention program for Senior Suites.
Minimum Requirements:
Graduate of an Accredited School of Nursing. Unencumbered RN license with the NDBON and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Two or more years of nursing experience is preferred. Previous geriatric experience preferred. Knowledgeable on basic care licensing regulations preferred.
ESSENTIAL FUNCTIONS, ROLES, AND RESPONSIBILITIES
- Responsible for understanding and enforcing the requirements of ND Basic Care Facilities. Knowledgeable on the criteria for basic care appropriate residents and makes arrangements for referrals, keeping Director of Senior Suites informed.
- Understands specific criteria for various pay sources and completes necessary procedures for each.
- Consistently documents according to nursing care plan as evidenced by:
- Nursing diagnosis and interventions addressing physical, psychosocial and safety.
- Expected outcomes for the resident as obtained from the resident assessment.
- Assists in updating care plans according to resident changes, condition, and physician’s orders and co-signed by an RN.
- Responsible for quarterly care conference assessment along with staff RNs and the Director.
- The documentation of the resident assessment will include the following:
- Skilled observation and documentation.
- Completes nurse assessment within 48 hours of return from hospitalization, including the assessment of psychosocial, physiological and safety factors affecting the resident.
- Knowledgeable on Service Agreement, Advanced Directives, Code level, Resident Bill of Rights, and admit transfer and discharge criteria.
- Knowledgeable of required resident forms.
- Consistently demonstrates ability and knowledge in performing basic nursing skills which include:
- Knowledge of the steps in an admission and discharge for Senior Suites.
- Communicate effectively with residents having special problems.
- Possess observation skills to assess the health and well-being of the residents.
- Knowledge of required paperwork to meet rules and regulations.
- Perform Skilled Nurse assessment.
- Knowledge of medication management.
- Knowledge of the terminal process and the ability to teach residents and families such in an attempt to avoid crisis.
- Procedure of death of resident in basic care.
- Schedules flu shots in the fall and obtains consents and orders.
- Supervises monthly foot care for residents and performs necessary foot care for diabetes along with staff RNs.
- Monitors monthly and weekly blood pressures and blood sugars and weights.
- Ensures that all new provider orders are complete, assisting staff RN as needed with orders as well as ensuring all required monthly forms are completed (i.e., medication records, care plans, caregiver skills task sheet, caregiver care plans, resident flow sheet, and other forms as required).
- Oversees medication set up and delivery system. Ensures MARs are ready for each resident monthly.
- Ensures that the caregiver skills task sheet and Care plans are ready for each resident monthly, updating them as required.
- Consistently demonstrates ability and knowledge in performing specialized skills which include, but are not limited to:
- Care of resident with a colostomy tubes, and Foley catheter.
- Acts as a resource for complex residents to incorporate other community support systems.
- Assists with setting up and providing orientation for new staff. Trains staff concerning functions and procedures as directed; provides staff in-services as required.
- Annually reviews department policies and procedures and assists the Director with updating policies and procedures.
- Assists with teaching and orientation of medication aides to meet the regulations of medication management. Works in conjunction with staff RNs to provide one-on-one teaching to medication aides on each shift until medication aide is proficient in med pass.
- Carries out tasks of the nursing supervisor in her absence.
OTHER DUTIES
- Assist Director with staffing including:
- Scheduling and rescheduling, finding replacements for ill calls and open shifts and filling shifts if no replacement can be found.
- Adjusting hours to meet the needs of Senior Suites.
- May be requested to complete infection preventionist program and oversee the infection prevention for Senior Suites. Responsibilities include:
- Works in collaboration with SMC Infection Preventionist.
- Completes infection control monitoring of residents.
- Teaches and monitors infection prevention processes to staff.
- Creates and updates policies on infection prevention as needed.
- Assist the Director with completing a mock survey as well as participating in health and life safety survey process.
- With the Director of Senior Suites, assesses prospective and current residents to determine care needs and appropriateness for residency. Assists with admission or readmission of new residents. Assists the Director of Senior Suites in developing and overseeing the implementation of resident care plans to comply with doctor orders and meet health care needs. Assures quality, continuity, and appropriateness of care provided.
- Evaluation:
- Consistently demonstrates the ability to evaluate and document the resident’s response to care given. Evaluates resident’s progressions, regressions, or status quo.
- Identify significant changes and/or abnormal findings and utilize appropriate problem-solving and intervention.
- Reviews documentation for all staff involved with each resident. Modifies nursing care plan as required by changing resident needs as determined by evaluation of care administered and established standards of care.
- Identifies at-risk residents and helps create risk management plan if needed.
- Communicates with residents, families, staff, physicians, and other healthcare providers regarding resident health issues and care plans.
- Participates in resident teaching. Consistently demonstrates implementation and documentation in an individualized care plan.
- Facilitates alternative placement or supplement care when appropriate.
- Responds to resident emergency calls as needed. Reviews and documents emergency call responses. Responsible for onsite needs if SMC nurse is unavailable or it is not feasible for the SMC nurse to assist.
- Ensures safe, secure storage of all medications, supplies, equipment, etc.
- Supervises nurse aides, med aides and staff RNs, as authorized by the Director.
- Assures accurate documentation and timely submission of services provided for resident billing.
- Manages and supervises other nursing functions as required.
- Other duties as assigned by the Director.
