This is an in person position located in Crownsville, MD.
Schedule: Monday - Friday (Flexible Dayshift hours between 7:30am - 6:30pm)

SUMMARY
The Case Manager will support clients and children by providing intake processing, community connections for social services, and other resources as needed and any discharge planning through the lifecycle of the client experience throughout time spent in Chrysalis House. We are looking for a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Foster a culture of recovery and peer support by cultivating positive and professional relationships with clients and engaging in peer interactions.
- Manage the entire client life-cycle from intake, through case management, to discharge planning for assigned caseload.
- Ensure timely completion of intake paperwork within 5 days of client arrival, and securely scan documents into the Electronic Medical Records (EMR) system, including admission letters.
- Send admission letters to probation agents and/or Maryland Behavioral Health Administrations (BHA) as necessary.
- Administer admission surveys, midterm evaluations, and exit surveys, forwarding completed surveys to the Performance Improvement Director.
- Coordinate and deliver care that is safe, effective, efficient, equitable, and client-centered.
- Manage case assignments, draft service plans, monitor progress, and determine case closure.
- Assist clients in achieving wellness and independence.
- Facilitate comprehensive care coordination, including information sharing and collaboration with multiple stakeholders.
- Advocate for clients by helping them make informed decisions about their clinical status and treatment options.
- Establish effective working relationships and collaborate with the medical team throughout the case management process.
- Coordinate benefits with social services, health agencies, and government entities.
- Engage proactively with patients to monitor progress and ensure satisfaction.
- Maintain accurate records of client information, complete required forms, and generate statistical reports.
- Advocate for quality and cost-effective interventions and outcomes.
- Adhere strictly to professional standards, protocols, rules, and regulations.
- Other duties as assigned.
QUALIFICATIONS
- Work cooperatively with co-Client Support Specialist, partner on issues and collaborate as needed.
- Understand clinical documentation pertaining to residential treatment, while maintaining HIPAA compliance.
- Accept and accomplish duties and goals with minimal supervision and have the ability to assist and support clients.
- Ability to problem solve through data analysis, integrate new ideas or processes, and maintain confidentiality.
- Proficient in Microsoft Office software platforms.
- Able to adapt to changes in work environment, manage competing demands, frequent changes, delays or unexpected events.
EDUCATION and/or EXPERIENCE
- BS degree in related healthcare field
- Proven working experience in case management, including as a Nurse, Medical, Mental Health Care Manager or a related job
- Excellent knowledge of case management principles, healthcare management and reimbursement
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem-solving skills and ability to multi-task
- Compassionate with teamwork skills
- Current license, certification or registration
- CPR, First Aid, and Med Certification preferred.
- Proficient in MS Word, Excel, PowerPoint, email-messaging tools, etc.
