Position Summary: The Registered Psychiatric Nurse (RPN) provides screening assessment and psychiatric nursing services on the Healthy Transitions Team (HT). This multi-disciplinary team is designed to provide outreach comprehensive evidence-based treatment referral and support services for a population of youth and young adults and their family members struggling with severe mental illness and co-occurring substance use. The goals of the Healthy Transitions Team Nurse are:
- To assist clients with meeting personal objectives for improving and maintaining mental health especially regarding medication management and compliance;
- Identify and provide intervention for youth and their families who are experiencing or at risk of developing substance use disorder;
- Promote, improve and sustain the overall wellness and physical health of the target population.
Qualifications:
- Associate or bachelor’s degree in Nursing.
- Minimum of two years experience working as a psychiatric nurse in a community-based behavioral health setting.
- Valid and current Rhode Island Registered Nurse License.
- Valid driver’s license and reliable transportation.
- Familiarity with screening/assessment tools, local care coordination resources, and evidence-based practices.
- Oral and written communication skills at a proficient level ensuring effective and professional communication.
- Ability to provide crisis intervention, brief counseling, or case management when determined necessary.
- Ability to provide psychiatric/medical consultation to HT staff and/or community providers as appropriate to position.
- Bilingual skills are compensated by an additional 6%, above base pay.
Physical Requirements: This position requires residential and community visits. Employees in this position must have the ability to:
- Travel to and from clients’ residences, community locations, and office site, which could include using walkways, stairs, and/or elevators.
- Lift up to 20 lbs.
- Work remotely and in person adhering to PHI requirements.
- Adhere to all vaccination requirements necessary to work in a healthcare setting.
