Minimum Requirements
- Experience in a health care setting preferred.
- Must be NY licensed as an LPN, and must maintain current and unencumbered licensure.
Salary Range: $27.50-$28.50 per hour depending on experience.
Responsibilities
- Take Temperature, Pulse, Respirations, and Blood Pressures and document them correctly.
- Perform nursing assessments on all move-ins and when there is a change in residents' condition, including head-to-toe and focal assessments.
- Proper assessment and documentation when resident returns from the hospital or rehab.
- Pass medications to all residents that are on medication administration.
- Order and refill medication for residents and their pharmacy.
- Fill out incident reports for falls, skin tears, medication errors, elopements, resident-to-resident altercations, and employee incidents.
- Complete all treatments per doctors’ orders.
- Update doctors about residents' condition.
- Properly fill out doctors’ orders and ensure they are charted, sent to pharmacy, and families notified.
- Update family members on residents' condition and any declines they may be having.
- Notify the Wellness Director and Executive Director when a resident is having a decline.
Charting Responsibilities
- Document residents' decline in condition.
- Record when a resident has a medication change.
- Document falls, skin tears, elopements, and altercations.
- Record aggressive behavior.
- Document when a resident is not eating or drinking.
- Record new orders.
- Hot box charting.
- Document treatments.
- Record when a resident is taken to the hospital or returns from being gone.
- Daily charting when a resident is on antibiotic therapy.
- Document when doctors or families are notified.
