Position Summary:
We have an exciting opportunity to join our team as a Field Nurse. In this role, the successful candidate provides safe, quality, therapeutic age-appropriate nursing care using evidence-based practice. The nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction.
Job Responsibilities:
- Performs other duties as assigned.
- Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age-related, environmental, spiritual, transpersonal, and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
- Derives the diagnoses and needs from the assessment data, validates the diagnoses, and collaborates with the patient, family, caregivers, and physician to develop an individualized plan of care to attain expected quality outcomes.
- Responsible for the coordination of care by interacting and collaborating with other members of the healthcare team to optimize patient outcomes, patient satisfaction, and healthcare goals.
- Delivers direct nursing care, ongoing assessment, teaching, and training to patients and caregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits using evidence-based nursing practice.
- Identifies deficits and barriers to effective knowledge understanding of education provided to patients. Employs strategies to optimize patient engagement. Demonstrates caring behaviors toward patients, family, and other caregivers.
- Completes and maintains accurate, complete, and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention, and evaluation according to Federal, Department of Health, and The Joint Commission requirements.
- Assists Agency leadership with achieving corporate goals, improving quality of patient care outcomes, and patient satisfaction. Participates in Agency quality assessment performance improvement initiatives.
Minimum Qualifications:
To qualify, you must have a Bachelor's degree in Nursing required for employees hired after 9/1/2020 (those who were hired prior to 9/1/2020 are grandfathered). Current and valid NYS Registered Professional Nurse license and registration. 1-2 years of experience in an acute care facility OR Home Health, Hospice, and Long Term Home Health Care Program. Excellent communication (both oral and written), interpersonal, and customer service skills. Critical thinking skills. Working knowledge of computers.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $104,685.60 - $129,435.60 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay, or other forms of compensation or benefits.
