Summary Description
Under the direct supervision of the Chief Medical Officer, serves as the primary care provider for all health center patients enrolled under his/her care. Responsible for the management of all medical care for his/her patients to include the provision of medical treatment and advice to patients as indicated. Arranges or causes to be arranged, secondary, tertiary, or specialized care as required; monitors patient progress directly or in concert with a consulting medical provider; responds to emergencies; encourages health maintenance; maintains appropriate records for accurate billing of all inpatient and outpatient procedures in a timely fashion. This position has Ryan White Title I related duties and responsibilities relative to the provision of care to persons living with HIV/AIDS.
Exemplary Duties/Responsibilities
- Provides and submits primary care services per appropriate medical specialty; secures case histories and related and/or pertinent information.
- Diagnoses and treats all patients following the appropriate examination; plans diagnostic studies for each patient as necessary; evaluates all diagnostic results.
- Completes all medical records and prepares medical reports as necessary.
- Supervises and directs assigned nursing personnel in the provision of nursing care and related procedures.
- Participates in or conducts team conferences/case studies/case management activities as necessary.
- Assists in the development of protocols and procedures for preventive and/or special services health education services and nutrition services as appropriate.
- Maintains professional competence through participation in continuing medical education and other appropriate learning experiences.
- Participates in and/or uses findings of research in medicine and related disciplines to improve the level of outpatient care at the Center.
- Attends regularly scheduled meetings of the health center's medical staff; participates on other medical committees as needed.
- Serves as preceptor for resident and medical professionals as reasonably assigned; participates in activities related to the development of health care plans as needed.
Patient Centered Medical Home - A patient-centered medical home is a model of care that strengthens the clinician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has a relationship with a primary care clinician who leads a team that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for appropriate care with other qualified clinicians.
Team Approach - A team-based approach to care is being emphasized by the health center and by the very nature of your position within the organization. Managing patient care is a team effort that involves each health center employee both clinical and nonclinical interacting with patients and working to achieve slated objectives.
Employment Standards
Education: Successful completion of four years of medical school and internship in an AOA or AMA certified program. Must be Board eligible or Board certified in a primary care discipline.
Experience: Entry level position acceptable; one-two years experience in community medicine preferred.
Knowledge: Good understanding of comprehensive health care programs and activities; interest in evaluation and planning activities desired. Basic knowledge of Physicians' Current Procedural Terminology (CPT) as defined by the American Medical Association highly desired.
Skills: Must have the ability to effectively communicate with physicians, patients, and staff; sensitivity to patient privacy and confidentiality; interest in evaluation and planning activities and a sensitivity toward access to care including patient/center access and/or patient to provider access.
Licenses: Must maintain a current license to practice medicine from the Virginia Board of Medicine, a current registration with the Drug Enforcement Agency, and possess an appropriate Virginia Driver's license, vehicle insurance, a good driving record and a willingness to utilize own vehicle as needed in connection with employment.
