Responsibilities
- Direct the overall operation of the clinical services within the Home Care Home Based (HCHB) system
- Provide oversight coordinating referrals to the agency with internal and external referral sources
- Direct and follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a home care setting
- Proactive, collaborative team member in the home care environment
Qualifications
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
- Two (2) years recent practical experience as an RN
- One (1) year experience in Home Health required.
