Position: Licensed Practical Nurse (LPN)
Willow Falls Senior Living is an industry-leading, independent, assisted living and memory care community in the heart of Crest Hill, Illinois. Our community is specifically designed to create a safe and stimulating environments for all our residents including those living with dementia. Our open concept encourages our residents to become involved in the daily rhythm of life in our community. As we continue to grow, we are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and contribute to the health and happiness of our residents.
Responsibilities:
- Conduct health assessments and create personalized care plans for each resident.
- Administer medications and treatments according to physician orders and established protocols.
- Monitor residents' vital signs and promptly address any changes in health status.
- Collaborate with interdisciplinary teams to provide holistic and person-centered care.
- Assist in the development and implementation of wellness programs and activities.
- Educate residents and their families on health-related topics and promote preventive care.
- Respond to medical emergencies and provide immediate and appropriate interventions.
- Maintain accurate and up-to-date resident medical records.
- Foster a supportive and compassionate environment for residents and their families.
- Participate in ongoing training and professional development opportunities.
Requirements
- You love working with people and handle everyday challenges with a smile!
- Graduate or Future Graduate from an accredited nursing program
- Hold a Current State License as a Licensed Practical Nurse
- Able to communicate well in both verbal and written forms
- Have a minimum of 1 year experience as an LPN in a long term care setting
- A natural problem solver
- Are committed to interacting professionally and with care toward residents, families, and other staff
