Summary:
Provides a high level of personal attention and concierge-style care to ensure clients can successfully navigate Spry and the healthcare system to fulfill their healthcare needs through strong communication and service. Delivers high-quality, high-touch primary care and care coordination in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures, and administrative policy. Builds relationships with patients, manages care plans, and performs outreach. Supports the Spry care team decision-making process, participates in weekly care team meetings, and generally coordinates clinical and auxiliary services to deliver a quality patient experience. Upholds the mission, vision, values, and customer service standards of The MetroHealth System and Spry.
Qualifications:
Required:
- Graduate of an accredited school of professional nursing (or a program seeking accreditation).
- Current Registered Nurse licensure in the State of Ohio.
- Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
- 2-5 years of healthcare experience as an RN, preferably in primary care, outpatient care, or care management.
- EMR knowledge and experience.
- Ability to work independently and as a member of a multidisciplinary team.
- Demonstrates excellent interpersonal, verbal/written communication, delegation, and problem-solving skills.
- Demonstrates strong analytical, data management, and computer skills.
- Demonstrates strong leadership, organizational, and time management skills.
- Demonstrates flexibility, energy, attention to detail, and problem-solving skills.
- Demonstrates excellence in customer experience in healthcare or hospitality industry.
- Desire to provide care to the people and the communities we serve.
- Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.
Preferred:
- BSN.
- Certification in a relevant nursing field.
- Knowledge of Epic EMR.
Physical Demands:
- May sit, stand, stoop, bend, and ambulate intermittently during the day.
- May need to sit or stand for extended periods.
- See in the normal visual range with or without correction.
- Hear in the normal audio range with or without correction.
- Ability to lift 250-300 pounds with assistance.
- Ability to push/pull up to 500 pounds with assistance.
- Requires some exposure to communicable diseases or body fluids.
- Requires some exposure to chemicals and cleaning products.
- Ability to use the computer.
