WesleyLife Hospice offers compassionate end-of-life care services across central Iowa. Our team collaborates with medical providers to create care plans prioritizing clients’ and families’ comfort, well-being, and dignity. WesleyLife Hospice operates 24/7, supporting and encouraging families so they can focus on meaningful moments.
WesleyLife Hospice is developing a palliative care program, and our Nurse Practitioner will be an important part of our planning team. The goal of the program is to improve quality of life by managing symptoms and providing emotional, spiritual, and practical support. Our Palliative Nurse Practitioner will help us:
- Develop protocols for assessing pain, symptom management, and emotional well-being, ensuring clients receive care tailored to their individual needs.
- Ensure team members receive specialized training in palliative care, communication, and end-of-life decision-making.
- Create pathways for seamless coordination between healthcare providers and ensure open communication with the client and family to align with their values and goals.
- Collaborate with local healthcare systems, religious organizations, and community groups to expand the reach of support services.
- Regularly assess the program's effectiveness through client outcomes, feedback, and team member evaluations to ensure the highest quality of care.
As our Palliative Nurse Practitioner, your regular duties will be:
- Travel anywhere WesleyLife Hospice serves clients to conduct thorough initial assessments of clients’ medical conditions.
- Develop individualized care plans in collaboration with clients, families, and other healthcare professionals.
- Perform advanced clinical procedures within the scope of practice.
- Educate clients and families about their medical conditions, treatment options, and self-care strategies.
- Provide compassionate and dignified palliative care and guidance to clients in home hospice settings.

Qualifications
Education
Required: Masters or better in Nursing.
Preferred: Doctorate or better in Nursing.
Experience
Required: Current certification as a Nurse Practitioner (NP) in Iowa, specializing in Family Practice, Adult-Gerontology, or Hospice and Palliative Care.
Required: 2 years of full-time clinical work experience as a Nurse Practitioner, preferably in home health, hospice, or palliative care settings.
