Overview
The Dartmouth Cancer Center is hiring for a breast cancer nurse navigator based in Lebanon, NH. The breast cancer nurse navigator would join a strong nursing team consisting of one other breast-specific navigator, triage nurse, and shared in-clinic nurse. The ideal candidate can navigate complex systems, help transition patients between surgery, radiation oncology, medical oncology, and support services and works well with provider partners in several areas. Position is primarily on site with the opportunity to work remotely 1-2 days a week.

In collaboration with a multidisciplinary team, the Nurse Navigator – RN serves as an advocate for patients with serious illness by providing care coordination, education and support to patients, families and caregivers throughout the care continuum to ensure high quality of care across all care delivery sites. Responsible and accountable for the management of resources and serves as a liaison between the patient and family and all physicians, internal and external healthcare providers, support network members and the wider healthcare community.
Responsibilities
- Assesses patient needs upon initial encounter and periodically throughout navigation, matching unmet needs with appropriate services and referrals and support services, such as dietitians, providers, social work, and financial services.
- Identifies potential and realized barriers to care (e.g., transportation, childcare, elder care, housing, language, culture, literacy, role disparity, psychosocial, employment, financial, insurance) and facilitates referrals as appropriate to mitigate barriers.
- Develops or utilizes appropriate assessment tools to promote a consistent, holistic plan of care.
- Facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and to promote continuity of care.
- Provides education to patient, families, and caregivers to the cancer healthcare system, multidisciplinary team member roles and available resources.
- Participates in the tracing of metrics and patient outcomes, in collaboration with administration, to document and evaluate outcomes of the navigation program and report findings to the cancer committee.
- Assists in the identification of candidates for molecular and/or genetic testing and counseling and facilitates appropriate referrals.
- Acts as a liaison between the patients, families and caregivers and the providers to optimize patient outcomes.
- Performs other duties as required or assigned.
Qualifications
- Graduate from an accredited Nursing Program required.
- Bachelor of Science Degree in Nursing (BSN) required; Master’s preferred.
- Three years of oncology experience in an acute or ambulatory care setting preferred.
- Strong organizational skills and the ability to prioritize and reprioritize.
- Ability to develop collaborative relationships internally and externally.
- Strong oncology knowledge.
- Basic computer skills.
- Ability to work autonomously.
Required Licensure/Certifications
- Licensed Registered Nurse with New Hampshire eligibility.
- Oncology Nurse Certification (OCN) required within one year of hire.
- Nurse Navigation Certification required within two years of hire.
