Home Healthcare: We provide physician-ordered skilled nursing and health care services to homebound patients recovering from illness or injury. We have worked with physicians and families in developing custom home care solutions for patients requiring specialists in wound care, diabetes management, cardiac rehabilitation, physical therapy, occupational therapy, speech therapy, nutritional assessments and dietary counseling.
ANX Hospice Care: ANX also provides 24/7 hospice care for individuals with limited life expectancies or for patients no longer willing to seek aggressive medical treatment. Hospice care is provided by our specially-trained team of health care professionals, including a medical director/physician, medical social worker, skilled nurse, home health aide, spiritual counselor and hospice volunteer.

Summary:
The Patient Care Coordinator will support the work of the Manager and is generally responsible for accomplishing all patient care related clerical procedures and ensuring compliance of all patient-related data.
Essential Job Functions/Responsibilities:
- Work with Home Health for any Hospice referral.
- Open charts as directed by the Director of Hospice or Associate Director of Hospice for any hospice referral; assure accuracy, completeness of chart.
- Screens telephone calls for Managers and ANX Hospice Clinical staff.
- Facilitates communication between patients/families and other team members.
- Takes and keeps a daily schedule for all team members. All staff schedule will be collected from all disciplines, make copies as needed, and fax schedule to facilities.
- Attends weekly team meeting and prepares the agenda and documents at team meeting; provide each discipline with necessary documents for them to bring to facility (schedules, med list for home and assisted living home patient, plan of cares).
- Establishes, maintains, and closes patient charts in an orderly fashion; this may include but is not limited to the mailing of and follow-up on all required documentation such as Plans-of-Care.
- Attending Physician Authorization forms and Medical Information Requests forms.
- Orders D.M.E as requested by facility or staff; ensure there is an MD order prior to ordering.
- Communicates with staff and outside agencies as directed by the Director of Hospice or Associate Director of Hospice.
- Does routine correspondence for the Hospice team.
- Ensure that patient consents are scanned within 5 working days; CTIs, POCs, are electronically signed by all MDs within one week of admission.
- Maintains A.O.T. and checks census daily for accuracy.
- Keep track of new admissions and needed F2F to be completed by a Hospice Physician or Nurse Practitioner; monitor doc tracking.
- Follow up with employees who have not submitted clinical documentation; inform the Director of Hospice or Associate Director of Hospice if the staff has not responded after 2nd reminder.
- Prepare documents for Dr. Angeles to sign for the team meeting and on-going throughout the day.
- Mail any Medi-Cal NOE before the 5th day of every month to the Medi-CAL office.
- Fax any required and necessary documents to Skilled Nursing Facilities (SNF) or homes (example: signed orders, copies of CTIs, POLST, POCs).
- Collect from nurses and track SNF monthly: Monthly Med ReCap and scan into EMR.
- Establish, maintain and close patient charts in an orderly and timely fashion and filed at the end of the day.
- Prepare agenda and organize documents for the weekly Interdisciplinary Team Meeting and assist Patient Care Manager in obtaining physician signatures.
- Assists in the maintenance of medical records, patient charts, and general filing.
- Perform special projects as assigned.
- Coordinate coverage with the Project Coordinator for lunch, team meetings, and PTO's/vacations.
- Check faxes throughout the day and communicate information to the team.
- Scan faxed documents in the chart.
- For any deaths: Follow with mortuary death attestation.
- For Attending Physicians besides Dr. H Angeles, inform the Attending Physician or Care Managed Group of the death of their patient and document in the EMR/Communication notes.
- Handles other tasks as assigned by the Director of Hospice or Associate Director of Hospice.
Qualifications:
- Minimum of high school education; preferably with secretarial and computer background.
- Must be able to report on-site to our Burlingame office.
- General knowledge of spelling, punctuation, grammar, medical records, and office procedures.
- Basic knowledge of office machines.
- Basic knowledge of telephone skills.
- Aptitude or computer data entry and use of current software systems.
- Ability to establish and maintain effective working relationships.
- Ability to meet the public and staff as a positive, friendly and professional representative of the agency.
- Effective social interaction, communication, and organizational skills and the ability to perform a variety of tasks efficiently.
