Home Base Inc. is seeking an ambitious, driven, and enthusiastic applicant for the role of Case Manager/Wrap Facilitator in our Behavioral Health Programs! The Case Manager/Wrap Facilitator position can be offered on a full-time basis and offers a competitive benefit package, including health, dental, vision, retirement, and paid leave.

Qualifications:
- Bachelor’s Degree in Social Work, Counseling, Sociology, or Psychology required.
- At least 2 years of experience in Case Management and/or Outpatient/Inpatient behavioral health setting preferred.
- 1-2 years of experience in leadership/management preferred.
- Must be able to communicate effectively and demonstrate experience.
- Computer skills are essential; must be comfortable with Outlook, Word, Excel, and PDF.
- Must have a valid driver’s license, current vehicle insurance, current vehicle registration, and inspection where applicable.
- Must be able to pass a NCIC/CIB background check and an APS/CPS Background Check.
- Must be able to provide three professional references.
- Experience with Telehealth platform preferred.
- Professionalism and customer service skills are essential.
- Written communication skills are essential.
- Must be able to work independently and without supervision at times.
- Must have strong communication and organizational skills.
- Must be empathetic, creative, reliable, and have strong skills when working with a team.
Responsibilities:
- Working under the Wraparound system of care, which emphasizes individualized, strength-based, trauma-informed, family-driven, community-based, and culturally sensitive care to the youth of West Virginia.
- Complete all required training within the first 30 days of employment.
- Act as a liaison between Counselors and Clients; making initial contact with clients.
- Must meet at least 5 hours/day or 25 hours/week billing quota.
- Ensuring client cases are scheduled within 24 hours of receipt of referral.
- Maintaining follow-up schedule of referrals and cases.
- Clinical administrative work deemed necessary by Wrap Facilitation Supervisor.
