Redwoods Post Acute is a skilled nursing facility with 152 beds located in the heart of Willow Glen in San Jose, California. We are looking for a compassionate and dedicated Occupational Therapist (OT) who can work collaboratively in a fast-paced environment. The ideal candidate will have excellent communication skills, a strong attention to detail, and a commitment to providing exceptional care to our residents.

RESPONSIBILITIES:
- Conduct comprehensive assessments to identify patients' strengths, limitations, and goals
- Develop individualized treatment plans based on the patient's needs and goals
- Provide therapeutic interventions to improve patients' physical, cognitive, and emotional well-being
- Collaborate with other healthcare professionals to ensure coordinated care
- Educate patients and their families on adaptive techniques and strategies for independent living
- Document patients' progress and update treatment plans as needed
- Stay current with advances in occupational therapy by attending professional development opportunities
REQUIREMENTS:
- A California license to practice Occupational Therapy is required
- BLS Certifications are required
- Is in good standing with all regulatory agencies and licensing boards is required
- Interest in working with the geriatric population
- Graduate of a program accredited by the American Occupational Therapy Association (AOTA)
- Knowledge of the nursing process and standards of care for skilled nursing facilities
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Excellent critical thinking and problem-solving abilities
- Basic computer skills and proficiency in electronic health records
EXPERIENCE:
- New passer/graduates are welcome
- With Skilled Nursing or Long-Term Care experience is preferred
- Experienced Occupational Therapist (OT)
