The Child Center Residential Treatment Facility (RTF) is a coeducational program in a warm, supportive environment that serves young people ages 12-18 who have had multiple psychiatric hospital placements or have been involved with the juvenile justice system. It is the only downstate RTF in the state that is a dual NYS Office of Mental Health/Office of Children and Family Services facility. We never consider it too late to give young people the help they need, and we are pleased to partner with these two agencies to ensure the young people in our care have the skills and support they need to achieve long-term success.
This role is within a Gated Facility and parking spot is provided.
Summary
The Psychiatric Nurse Practitioner (PMHNP) provides services to RTF residents and BH clients, including evaluations, consultations, advisements, etc.
Essential Functions:
- Identifies, evaluates, and addresses psychiatric and/or substance abuse behavioral health issues.
- Conducts psychiatric assessment and medication management reviews of all enrolled clients; coordinating and referring for specialized care when indicated.
- Provides psychoeducation to clients and/or families about preventive care, substance abuse, psychiatric or medical issues, and use of prescribed medications with their possible side effects.
- Documents all psychiatric and/or substance abuse evaluations, diagnoses, procedures, treatment, outcomes, education & referrals consistent with state and city regulatory standards.
- Conduct medication management sessions and prescribes appropriate medication with corresponding documentation in the EMR
- Signs treatment plans in a timely manner.
- Resolving any off-shift issues with the pharmacy or communications with outside facilities (ER collateral, potential referrals, etc)
- Attends meetings including treatment conference, medical and program staff meetings, team meetings.
- Provides coverage for psychiatric providers, when coverage is needed, as part of larger team.
- Coordinates and consults with treatment team to ensure continuity of care and best practice.
- Participates/educates in learning series to enhance mental health practice throughout organization.
- Performs other related duties as assigned.
Minimum Qualifications:
- Board certification as a Psychiatric Nurse Practitioner
- DEA Certification and NYS License required
- Experience prescribing and managing clozapine
- Extensive training and/or experience in mental health and substance abuse treatment.
- Knowledge of insurance/managed care process.
- Experience in working with children, adolescents, and young adults.
- 3 years experience as a Psychiatric Nurse Practitioner
