Position Mission:
The Care Manager plays a key role in managing the medical and cognitive challenges of aging, ensuring that each client’s dignity and independence are maintained. Through strategic advocacy and collaboration with a care team, you will provide personalized support that brings peace of mind to families and optimizes client well-being.
Key Responsibilities:
- Comprehensive Care Management Assessments
- Conduct assessments covering medical, social, financial, psychological, and personal care needs.
- Identify and document opportunities to enhance the client’s quality of life.
- Holistic Care Planning
- Develop strategic and creative care plans based on client needs and preferences.
- Implement and adjust care plans to meet changing client conditions.
- Monitoring and Reporting
- Evaluate the effectiveness of care plans and provide detailed updates to the care team and family members.
- Resource Coordination and Advocacy
- Identify and manage personal and medical care resources, advocating for client services across various providers.
- Ensure continuity of care by coordinating with medical, legal, and community resources.
- Family Support
- Provide coaching, mediation, and healthcare navigation to ease the emotional burden of caregiving for families.
- Assist in facilitating difficult conversations and guide families in making informed healthcare decisions.

Preferred Qualifications:
- Certified Care Manager (CMC) or Certified Case Manager (CCM)
- 5+ years’ experience in community, hospital, hospice, or home health case management
- Bachelor’s or Master’s degree in Healthcare or related field (RN, PT, OT, PA, LCSW license preferred)
- 2+ years’ experience in case management or healthcare administration
- Strong computer skills (Email, Word, Excel, Internet)
- Valid driver’s license and ability to travel locally to client homes
