Position Summary:
Provides the initial and ongoing assessment and treatment of patients, ensuring appropriate initiation of care plans, services/ levels, and OASIS assessment information that accurately reflects the patient's clinical, functional, and psychosocial needs.
Responsibilities
Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:
- Develops care plans appropriate to the patients and patient family's health care needs in conjunction with patient, family, physician, clinical operations team, and third-party payers including obtaining medical orders and approval of medical services.
- Completes the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient.
- Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.
- Communicates to the health care team members the outcome of the assessment/reassessment (if applicable) visit to ensure appropriate follow up occurs based on the needs of the patient/family's plan of care.
- Ensures problem resolution at the point of contact, elevates when appropriate.
- Maintains appropriate productivity requirements.
- Other duties as assigned.

Qualifications
Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
- Education: New York State Licensed RN
- Experience: One year experience as a clinical pediatric nurse required, home care experience preferred.
Licensure, Certification, and/or Credential Requirements:
- New York State Licensed RN
Knowledge, Skill, & Ability Requirements:
- Working knowledge of health care reimbursement systems, government regulations and health industry accreditation standards relative to health care eligibility criteria, payer utilization, prior authorizations, and assessment/teaching standards.
- Working knowledge of IVs and advanced wound care products.
- Excellent organizational/interpersonal skills (including time management, patient visit prioritization).
- Ability to effectively assess the physical and psychosocial needs of the patient/family and develop an appropriate plan of care.
- Displays an exemplary level of patience, courtesy, and flexibility.
Technical & Systems/Equipment Proficiencies:
- Microsoft Office Suite: Outlook, Word, Power Point and Excel
- E-Record (electronic medical record systems)
- Office equipment, such as computers, printers, copiers, and fax machines, as well as multi-line phone systems
- Intranet/Internet
- Willing to utilize the technology as appropriate to meet the requirements of the job functions.

- Travel throughout service area year around.
