What would you do as a RN - Manager of Community Support and Population Health
Playing a pivotal role in enhancing health outcomes for the communities we serve, this position is responsible for meticulously planning, organizing, and implementing clinical activities tailored to support community health initiatives. Utilizing advanced population health management tools, including clinical disease guidelines and care gap analyses, the manager designs innovative work plans that directly address significant health metrics. Collaboration is key, as this role works closely with clinical and quality staff to optimize quality outcomes on both individual and population levels.
You'll assess the overall population for improvement opportunities, analyzing data trends to identify gaps in clinical care and high-risk patient conditions. By employing the nursing process and best practices, this role aims to improve chronic condition management while facilitating community partnerships to strengthen resources, thereby driving continuous improvement within the healthcare system. Ultimately, the RN Population Health Manager is instrumental in fostering a healthier, more informed community.
What matters most
To thrive as the Manager of Community Support and Population Health, candidates must possess a Bachelor of Science in Nursing and an active New York State RN license. A robust clinical background is essential, with at least five years of experience in an acute care setting, alongside five years in a supervisory healthcare role. Proficiency in Microsoft Excel, PowerPoint, and Word is required to effectively manage reports and presentations. A comprehensive understanding of Medicaid and Medicare regulations is critical to ensure compliance and improve health outcomes.
