The Wound Care Nurse is responsible for developing and ensuring implementation of plans of care that are comprehensive and patient specific, in collaboration with the Interdisciplinary Team. The Wound Care Nurse will be responsible for establishing an evidence-based Wound Care Program at Ferncliff Nursing Home and serving as an expert in the management of wounds and promotion of education and excellence in this area. Furthermore, the Wound Care Nurse will be a resource for patients and their families, providing comfort and support. He or she will also collaborate with medical and nursing leadership to promote institutional standards of care and monitor relevant data and benchmarks.
Responsibilities
- Preserving integrity of skin by implementing and monitoring skin prevention strategies.
- Ensuring all wound and skin care protocols are followed. Organize multidisciplinary wound and skin rounds on a weekly basis for high-risk patients; patients with current pressure ulcers and other significant wounds.
- Assist Wound MD with all patient rounds.
- Providing hands-on wound care for all difficult and non-healing wounds and formulating patient-specific care plans.
- Serves as a resource for facility staff on all issues related to wound and skin care.
- Provides In-Services and Wound Care Competencies to Care Members on all shifts on an ongoing basis.
- Participates in the Bi-Annual Skills Fair; assists Staff Development throughout the Skills Fair.
- Records timely and accurate documentation weekly on tracking; push tools; care plans and Wound Care Notes.
- Notifies CNO/ADON of any facility acquired areas weekly for RCA to be completed.
- Follows Archcare Wound Care Formulary Products.
- Develops and implements standards of care, policies, and procedures, particularly in problematic areas or areas of ambiguity.
- Reports to facility QAPI program related to wound program such as healing rate and or barriers to healing.
Qualifications
- Minimum 3-5 years clinical experience as a registered nurse in the acute care/sub-acute care setting.
- Minimum one (1) year experience in wound care.
- Current state licensure as a Registered Nurse.
- Wound Care Certification WCC/ WOCN preferred.
