Adecco Healthcare & Life Sciences is hiring for an LPN Case Manager in Kissimmee, FL! This is an in-person position.
Type: Contract – possibility of extension or perm offer
Schedule: 8 am to 5:00pm M-F
Pay: $30-32.80/hour
Location: Kissimmee, FL
Responsibilities of the LPN Case Manager include:
- Discharge planning
- Liaison between patient, family and provider
- Arranging referral as needed
- Evaluate the ongoing recovery of patients after discharge
- Accurately and completely update patient records
Requirements Include:
- HS Diploma or GED
- Graduation from an accredited LPN program/Associate degree
- Florida State LPN licensure – active and in good standing
- Chart review experience
- Outstanding time management skills
- Great communication skills
- Strong documentation skills
