Primary Purpose:
We are seeking a highly skilled and compassionate certified Nurse Practitioner to join our Advantmed provider network. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care. This is a full-time position with opportunities to grow within a healthy company.
NP Responsibilities:
- Perform annual in-home wellness visits and risk adjustment health assessments on a population with chronic conditions including comprehensive assessments, physical examinations, vital signs, medication history reviews, cognitive assessments, and more.
- Provide individualized quality care to the elderly population in their homes.
- Deliver patient health education on topics such as pain management and medication.
- Build relationships of trust with members through exceptional communication and empathy.
- Assist in closing quality care gaps (i.e., screenings and labs).
- Collaborate to enhance care delivery.
- Travel throughout your assigned state.
- Day-to-day responsibilities will be within a 55-mile maximum radius.
- Other duties as assigned.
Locations:
NP Qualifications:
- Valid unencumbered NP License for the state you will be working in.
- Travel within the assigned job posting county up to a maximum 55-mile radius.
- Previous in-home risk assessment experience preferred.
- 3 years patient care experience preferred (primary care/adult/geriatric, EMR).
- Access to reliable transportation to travel to member's homes.
- Strong ability to work within our EMR system.
- Ability to work independently.
- Bilingual is a plus.
