We are hiring a Physical Therapist to join our dynamic care team at The Tremont Rehabilitation and Skilled Care Center in Wareham!

As a Physical Therapist, you will provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload while participating in the clinical team process. You will also provide teaching and training to caregiver staff and supervise other therapists, assistants, aides, and students as required.
Education & Experience:
- Bachelor of Science Degree, Master of Science, or Doctoral degree in Physical Therapy.
- Appropriate licensing requirements by individual states must be maintained.
- Two (2) years of clinical experience preferred.
Duties & Responsibilities:
- Screen patients/residents.
- Evaluate patients/residents and develop appropriate care plans.
- Perform assigned administrative duties like assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
- Under a physician’s order, provide direct resident treatment in accordance with an established plan of care.
- Contribute to team meetings and decision-making processes.
- Consult with and train patients, residents, and caregivers.
- Monitor efficiencies on an ongoing basis in collaboration with supervisors.
- Document activities according to the company’s clinical documentation system on a timely basis.
- Demonstrate knowledge and application of Medicare and other reimbursement regulations.
- Communicate effectively with patients, residents, team members, customers, and supervisors.
- Provide appropriate consultation to the facility.
- Participate in clinical program development and implementation.
- Participate in the training and orientation of other staff, students, and volunteers.
- Participate in the interview process for new staff.
- Participate in census building and marketing activities.
- Participate in quality improvement activities.
- Ensure the delivery of quality services and outcomes.
- Comply with facility Privacy Practices policies and procedures.
- Exhibit courtesy, compassion, and respect to all.
