Job Summary: Provide clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed in Day Center.

Duties/Responsibilities
Essential Job Functions:
- Complete participant evaluations per PACE schedule (new/reassessment & change in condition) and develop treatment plans. Evaluations include but are not limited to the following: gait assessment; balance; bed mobility; functional status; physical status and wheelchair/assistive device needs.
- Administer physical therapy treatments to participants, evaluate effectiveness of interventions and modify services as appropriate.
- Integrate the physical therapy treatment plan into the development of the Plan of Care (POC) for maximum holistic gain for participant.
- Function as a member of the interdisciplinary team and participate in case conferences when necessary.
- Assess the need for, recommend, order, distribute and maintain wheelchairs and assistive devices for participants.
- Develop therapeutic exercise/wellness programs in accordance with participant needs.
- Provide training to caregivers and PACE staff members for those parts of the treatment plan that can be carried out by them.
- Provide supervision to program aides and other staff who are implementing wellness program.
- Communicate participant changes to team members.
- Participate in the development of the IDT Plan of Care.
- Participate in the coordination of a 24 hour care delivery.
- Complete all documentation in the medical record as required.
Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Sitting, Standing, Walking, Talking, sitting at a desk, and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands: Sedentary physical activity performing non-strenuous daily activities of an administrative nature.
- Stand less than 33% of time on duty.
- Walk less than 33% of the time on duty.
- Sit between 33% and 66% of time on duty.
- Use of hands to finger, handle or feel over 66% of time on duty.
- Reach with hands or arms less than 33% of time.
- Full ability to talk and hear.
- Correctable vision required.
- Able to lift/carry up to 15 lbs.
Minimum Educational and General Qualifications: Bachelor of Science or Master Degree in Physical Therapy. Graduate of a physical therapy program accredited by the American Physical Therapy Association. Licensed to practice physical therapy in Louisiana. At least one year of general physical therapy experience in a geriatric setting. Must possess a valid driver's license and reliable transportation.
All candidates for employment with CCANO must submit to a criminal records check as a condition of employment. New employees will be required to complete a criminal background check prior to employment. Compliance with the State of LA Child Protection Act and Archdiocese Policy on Abuse and Neglect, to provide for the safety and security of all clients served by CCANO programs. Certain programs require additional background checks in addition to the Archdiocesan check. These may include the "State Police Check" and "The Department of Education".
