North Star Health is a not-for-profit, community-based health care system that serves southeastern Vermont and southwestern New Hampshire. The unique model of our health system integrates 6 federally qualified health centers, including two dental clinics, a vision center, five school-based health centers and a site within our local recreation center. Our organization works collaboratively with Springfield Hospital to meet the health needs of our communities.
Our health centers are Medical Home sites that hold the highest quality certification attainable (Level 3) from the National Committee for Quality Assurance. Patient-centered care is evidence-based, provides high-quality outcomes, and is accessible and affordable to all based on a sliding fee scale. North Star Health is in search of a clinical social worker to join our expanding behavioral health program:
- Provide primary care behavioral health consultations and short-term interventions.
- Incorporate behavioral health interventions based on skills-based treatment and facilitation of behavior change in a population working to better manage, reverse, and prevent chronic conditions.
- Demonstrate professional ethics and behavior and maintain confidentiality at all times.
- Maintain professional competence through remaining abreast of current research, legal requirements, evidence-based practices, and technology.
- Consultation and contact with North Star School Based Health Centers, outside agencies, and schools.
- Conduct risk assessments (risk to harm of self and others) and collaborate with medical providers as needed.
- Execute responsibilities efficiently and in an organized manner, including maintaining electronic medical records.
Requirements
LICSW, LCMHC or providers working towards licensure eligible.
