Position Summary: As part of the Trauma System Therapy-Community Health Team (TST-CHT), the Peer Support Specialist will work collaboratively with FSRIs team members, Community Health Workers and Behavioral Health Clinicians. The Peer Support Specialist will assist with a patient's needs and well-being through individual goal setting, advocacy, mentorship, education, and addressing/ameliorating social determinant of health barriers. The Peer Support Specialist may have lived experience which may come from individual or family experiences/interactions with a variety of systems.
Qualifications:
- Associates or Bachelor's degree preferred, High School Diploma required.
- Bilingual in English and Spanish preferred.
- Cultural sensitivity necessary to work with a diverse client population.
- Knowledge of computers and medical records required.
- Possession of a valid driver's license, reliable transportation and auto insurance is also required.
- Complete necessary immunizations needed to work in a health care setting.
- Bilingual skills are compensated by an additional 6%, above base pay.
