Job Summary
Job incumbent provides coordination of care and ancillary services to clients enrolled into the Healthy Transitions Project. Healthy Transitions is a SAMHSA funded grant intended to serve young people ages 16-25 who may need support in their transition to adulthood. This project will support evidence-based strategies and interventions to build resiliency and skill development. Adhere to the TCC Ch 'eghwtsen' model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.
Essential Functions

Representative Duties:
- Provide individual and family case management for clients in coordination with other providers including the Directing Clinician.
- Provide outreach services to clients including routine phone and emergency outreach.
- Network with area professionals to ensure follow-up care and referrals, including transfers of care between Psychiatry, Therapy, Inpatient and Outpatient care, Primary Care clinics and Specialty Care clinics.
- Provide coordination and support for assessments, using DSM (Diagnostic and Statistical Manual of Mental Disorders) and ASAM (American Society of Addiction Medicine) criteria. Coordinate directly with other community agencies on inpatient and outpatient referrals for behavioral health treatment.
- Assist with urgent and routine requests for care, including community and provider requests, gathering pertinent history and records, and assisting with triage, in collaboration with Direction Clinician, Psychiatrists and Medical Providers.
- Provide prevention, psychoeducation, and support services to patients regarding mental health, substance abuse, and wellness with particular attention to challenges associated with transitional age youth.
- Provide and receive appropriate referrals to and from other agencies and resources, including but not limited to: Chief Andrew Isaac Health Center, Fairbanks Memorial Hospital, Anchorage Native Medical Center, Fairbanks community providers, external laboratories/testing services/imaging centers, inpatient and outpatient services both in and out of state.
- Assist with letters, written evaluations, care summaries and community resource agencies as needed, to provide additional patient and provider support.
- Participate in regular clinical supervision with Clinical Supervisor.
- Complete documentation in a timely manner in accordance with guidelines established by Tanana Chiefs Conference, Indian Health Services, state and federal agencies and regulatory agencies.

Minimum Qualifications
- Bachelor’s Degree from an accredited university in psychology, counseling, human development, or related medical or behavioral health field. Applicants without the educational requirements must have a minimum of four (4) years of experience as a behavioral health case manager or coordinator.
- Must have direct patient care experience, including work with children, adolescents or families.
- One year work experience in primary care or behavioral health, addictions, community case management or similar. Experience working in a tribal health organization is preferred.
- Experience working with Alaska Native persons and familiarity with rural village dynamics.
- Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements and State of Alaska background check.
Knowledge, Skills and Abilities:
- Experience and education regarding provision of a variety of medical and behavioral health services to adults, children, and adolescents, to include familiarity with: service provision, DSM diagnosis, severely emotionally disturbed children, chronic mental illness, dual diagnosis and integrated treatment for substance abuse and mental illness, treatment planning, treatment coordination, case management, consultation, routine charting, Medicaid regulations, and professional reports. Familiarity with Fairbanks area, Interior Regional, and statewide health services in mental health, medical treatment and substance treatment.
- General knowledge of case management practices, including familiarity with relevant programs and services.
- Strong interpersonal, written and verbal communication skills with clients, providers, and other stakeholders.
- Staff must demonstrate the ability to assess and respond to client needs in a timely manner.
- Strong critical thinking, prioritizing, and problem-solving skills.
- Ability to establish and maintain effective working relationships with those contacted in the course of work, including service representatives and the general public.
- Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
Supervision: This position has no direct supervisory responsibilities.
Physical demands: Ability to lift 50 pounds, possible exposure to a variety of infectious and communicable diseases. Traveling from site to site (includes travel in small aircraft or boat), standing, and stooping for extended periods of time. Exposure to a variety of working environments, including rural village communities. Must be able to work in a busy, and sometimes stressful, clinic environment.
Summation: Incumbent will work collaboratively with Behavioral Health staff and beneficiaries with providing assistance to engage in addictions treatment services. This position requires knowledge and experience in working in remote areas, with primarily Alaska Native clientele and their families.
