POSITION DESCRIPTION:
The Float NP will provide coverage at SNFs in various locations throughout the state of South Carolina. Most facilities are in the greater Charleston / Columbia area and the new provider must reside within the state of South Carolina.
The Float NP will provide as-needed coverage to geriatric patients in SNFs. The Float NP will not be permanently assigned to one facility but will float within the designated market for purposes of temporarily filling vacancies, sick leave coverage, etc.
Responsibilities:
- Effectively medically managing enrolled SNF patients
- Conducting comprehensive assessments
- Developing and delivering individualized plans of care
- Attending facility care team meetings
- Communicating with family members and caregivers
- Ensuring the care team understands the member’s goals of care and needs
As part of the clinical team, the Float NP will be supported by the Clinical Manager and/or Director, as well as the local Medical Director.
The organization uses an EHR.
While most of the SNFs are located in the greater Columbia and Charleston areas, some statewide travel to other territories will be required, including possible overnight travel.
REQUIREMENTS:- South Carolina NP license
- DEA
- 1+ year advanced practice experience
- 1+ year geriatric care experience
- 1+ year of experience in SNFs or similar environments
