Position Summary
We are seeking a dedicated and compassionate Case Manager to join our team. Operating within our organization's fervent mission to provide comprehensive support, the Case Manager will play a crucial role in delivering highly personalized and empathetic assistance to individuals seeking help through our Benefits Navigation and Income Support services. In this pivotal role, you will be collaborating closely with our valued clients to comprehensively assess their unique needs, deftly navigate benefits eligibility processes, and expertly connect them with the most suitable and beneficial resources available.
Key Responsibilities:
Benefits Navigation:
- Conduct benefits eligibility screenings to help clients determine their eligibility for various benefits.
- Provide guidance and support to clients in the application and access of benefits.
- Facilitate connections to services that improve clients' health, economic, and social well-being.
Income Support:
- Assess clients' financial needs and offer emergency/one-time financial assistance for urgent situations.
- Assist clients by facilitating utility bill payments through our Utility Bill Payment Assistance program.
Client Support and Advocacy:
- Establish and maintain positive relationships with clients, providing ongoing support and follow-up.
- Advocate for clients to ensure they receive the necessary services and support.
- Maintain accurate and confidential records of client interactions and services provided.
Collaboration and Coordination:
- Collaborate closely with community organizations, service providers, and other stakeholders to expand the available resources for clients.
- Coordinate with internal team members to ensure comprehensive service delivery and address any gaps in client support.
Program Improvement:
- Contribute to the continuous improvement of program services by providing feedback and actively participating in team meetings.
- Stay updated about changes in community resources and benefits programs to effectively support clients.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in social work, psychology, counseling, or a related field
- At least 1 year of relevant experience in case management or a related role
- Knowledge of mental health and substance abuse issues
- Strong assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively with diverse populations and community partners
- Proficiency in case management software and computer applications
- Valid driver's license and reliable transportation
- Ability to maintain client confidentiality and adhere to ethical guidelines
The salary for this position ranges from $22.00 to $28.00, depending on experience.
