Position Summary: We are seeking a Senior Nurse Navigator to lead daily operations of our bundled payment program. In this role, you will collaborate with the patient care team to manage BPCI patients from inpatient stays, ensuring the appropriate next site of care and discharge disposition throughout a 90-day episode. You will engage patients early, assess readmission risks, and develop tailored transition plans to enhance patient outcomes and experiences while reducing costs.
Education Requirements:
- Valid RN license; BSN required, Master's preferred.
- 3-5 years of case management experience in hospital or community settings.
- Strong understanding of the Bundled Payment Care Improvement program.
Experience Requirements:
- Comfortable in a collaborative, shared leadership environment.
- Excellent verbal and written communication and presentation skills.
- Advanced analytical skills and the ability to prioritize multiple projects and demands.
- Proficient in Microsoft Office (Word, Excel, PowerPoint).
- Valid driver’s license and willingness to travel for patient visits.
Key Responsibilities:
- Lead and manage the bundle program operations, ensuring compliance with BPCI guidelines.
- Identify and engage patients early in their hospital stay to initiate the onboarding process.
- Collaborate with multidisciplinary teams to conduct daily rounds and gather clinical information to inform discharge planning.
- Assess patients for readmission risks and develop site-specific transition plans with appropriate support systems to minimize readmissions.
- Maintain and manage the census of BPCI patients using Episode Connect and other reporting tools.
- Facilitate communication between patients, families, and the Principal Accountable Practitioner to confirm discharge plans and ensure a smooth transition of care.
- Document all patient assessments, onboarding processes, and transitions of care, maintaining accurate records for the full 90-day episode.
- Support BPCI work teams by reporting on key metrics such as length of stay and next site of care, collaborating with colleagues to drive program success.
Pay Range: $31.50-$45.70
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
