An Overview
The Licensed Practical Nurse (LPN) will be responsible for all nursing duties related to successful attestation of Meaningful Use and Primary Health Care Screenings objectives. The LPN will work under the supervision of the Medical Director and in coordination with the Psychiatric Nurses and the Pharmacy Technicians. The LPN will be responsible for conducting all required nursing activities to meet Meaningful Use and Primary Health Care Screenings expectations, document findings into the electronic health record, and effectively communicate with the psychiatrists and other treatment team members.
What You Should Bring
The ideal candidate should have thorough knowledge of the theories, techniques and practices of community mental health nursing; thorough knowledge of and ability to provide primary health care screenings; ability to work with individuals who have multiple impairments; ability to accurately collect and enter health care data into an Electronic Health Records (EHR); ability to establish and maintain effective working relationships with physicians, nurses, patients, community providers and other treatment team members; and the ability to communicate clearly and effectively, both orally and in writing.
The Opportunity
- Conducts primary health care screenings for identified patients on a timely basis.
- Meets with patients prior to psychiatrist visit to take vital signs including height, weight, BMI, blood pressure and pulse rate and documents in the Electronic Health Records (EHR).
- Takes a basic patient history including medical conditions, medications, allergies and pregnancy status when applicable and documents in the Electronic Health Records (EHR).
- Assists patients with completing a depression screening tool.
- Reviews laboratory reports and enters lab values in the Electronic Health Records (EHR).
- Inquires at each visit if there have been changes in medical conditions and medications and documents in the Electronic Health Records (EHR).
- Obtains preferred pharmacy information and documents in the Electronic Health Records (EHR).
- Reviews summary of care reports from external providers and documents information in the EHR.
- Ensure that all medical documents are sent to medical records for processing.
- Notifies psychiatrists and nursing staff of any abnormal results in a timely manner.
- Ensures patient flow by seeing patients in a timely and efficient manner.
- Assists with chaperoning patients as needed.
- Provides health education in conjunction with the psychiatrist and nursing staff.
- Performs other duties as assigned.

Minimum Requirements
- High school Diploma or equivalent.
- Graduate of a state board approved practical nursing program.
- Licensed as an LPN by the Commonwealth of Virginia.
- Completion of college courses in psychiatric or community health nursing.
- At least one year of experience in nursing case management.
- Fluent and Bilingual English / Spanish or Amharic.
This position requires the successful completion of pre-employment background checks including but not limited to: FBI/Federal Records Check; VA State Child Abuse/Neglect Registry; Medicare/Medicaid Fraud Database; education/certifications; drug screening; and driver’s license.
