CASE MANAGEMENT:
- Provide direct case management services to clients for services, including but not limited to: educational and vocational pursuits, legal issues, life skills (e.g. resume writing, budgeting, etc.), advocacy, and accessing housing, social services, and other community resources.
- Assist with the recruitment and screening of eligible clients to retain services through the START Program.
- Coordinate and facilitate referral process and intake session.
- Prepare and submit reports as necessary to referral sources and community agencies including DCP&P and substance use treatment providers.
- Generate outreach letters to clients regarding non-attendance and potential discharge.
- Coordinate intake sessions and schedule initial clinical assessment sessions. Administer case management assessments and assist with completing clinical screens as appropriate.
- With client participation, and in collaboration with other START providers, develop and update client treatment plans to include interventions consistent with ARC and the Nurtured Heart Approach.
- Complete case management documentation in a clear, concise, accurate and timely manner.
- Assist with the collection and documentation of urine drug screens as needed.
- Maintain ongoing relationships with various referral sources and relevant community organizations.
- Participate in program promotion, marketing, and community outreach efforts.
- Participate in program, committee and clinical staff meetings and training program(s) as indicated.
- Provide support to ensure seamless transitions among CGE’s continuum of care as needed.
- Provide proper crisis management/intervention as needed and notify supervisor as soon as practical.
- Exercise independent judgment when observing and assessing a client’s current functioning.
- Respond to clients in an attuned and effective manner, employing CGE’s protocols and procedures as appropriate to presenting situation.
- Make appropriate judgments with respect to life threatening circumstances and take appropriate action.
- Participate in program evaluation and ongoing quality improvement efforts.
- Prepare and deliver oral and written reports to staff and appropriate professional agencies as indicated.
GENERAL DUTIES:
- Build a network of support within the program and among clients that focuses on trust, well-being and social/community integration.
- Maintain records as required by organizational policies and regulatory and accrediting agencies.
- Work as a member of a team to establish and maintain a high level of care and respect for, and communication with clients.
- Work in a consistently professional manner at all times, which includes, but is not limited to, treating all residents, staff, guests, and volunteers with dignity and respect.
- Remain flexible and available for day and evening hours.
- Perform other duties as requested.
Requirements
QUALIFICATIONS:
- Master’s degree in human services, social work, or psychology or currently enrolled in a graduate level program.
- Minimum of two (2) year of experience working with substance use and co-occurring disorders and or case management.
- Bilingual (English/Spanish) speaking a plus.
- Clean driving record; comfortable transporting clients and children.
- Ability to work independently and handle crisis situations.
- Excellent organizational and interpersonal skills with friendly and professional demeanor.
- Current NJ driver's license needed; will be required to meet safe driving requirements.
- Ability to be flexible to address daily changing needs within the community, and to handle obstacles with compassion and resolve.
