The Physician is an essential member of a multidisciplinary team that provides health care to patients in an urban community health center serving a diverse community. Broadly, she/he will be a key player in developing and supporting the infrastructure to implement a preventive and chronic care model of team-based care delivery, with a patient-centered and continuous improvement perspective.

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.
QUALIFICATIONS:
Education:
Required – MD/DO, completion of an accredited residency in Internal Medicine or Family Medicine, Board certified or eligible.
Experience: Required: Residency as above, Bilingual Spanish preferred.
Knowledge/Skills:
- Excellent oral and written communication skills.
- Strong organizational skills.
- Great self-starter who integrates with multi-disciplinary team.
- Motivational skills with patients.
- Basic electronic documentation skills.
License/Registration/Certification:
Active Massachusetts licensure; DEA; Massachusetts controlled license. Board eligibility. Basic Life Support within 90 days of start.
Responsibilities:
- Clinical Care: Care for patients as part of the care team of physicians, mid-level providers, residents, nurses, medical assistants, social workers, nutritionists, and non-clinical administrative staff in each of the primary care clinics.
- Laboratory and Image Review: Review and sign off all laboratory and imaging results from patients.
- Referrals/Transition of Care: Review and sign off all transition of care documentation (ED, hospital, referrals) from patients.
- Access: Accessible for clinical care both in person and after hour’s phone consultation.
- Documentation: Clearly document all clinical interactions in the electronic health record.
- Patient Centeredness: Ensure that the patients’ needs are at the center of all treatment plans.
- Community Member/Citizenship: Self-monitoring and team monitoring to enhance skills.
- Continuous Quality Improvement: Accountable for individual and team quality measures.
- Participation in Clinic Specific Projects: Might include internal improvement projects, research projects, and grants.

- Takes responsibility for delivery of services.
- Takes initiative in making suggestions for improvement.
- Meets deadlines without causing disruption.
- Utilizes resources efficiently.
