Trellis Health is seeking either a Nurse Practitioner or Physician Assistant for their current APP opening!
The APP hired for this position will be expected to meet all requirements necessary for working independently in the state of Maine.
Position Summary

As part of a start-up subsidiary of Maine Health, you will play a crucial role in shaping our clinical practices and patient experiences from the ground up. This position requires flexibility, a high level of autonomy, and a proactive approach to both problem-solving and patient care.
The APP manages a comprehensive patient panel as a key member of the care team, fostering trust-based relationships and proactive healthcare management. They collaborate closely with other team members, including nurses, medical assistants, and specialists, to develop, update, and prioritize patient care plans. The APP ensures team-driven adherence to these plans, focusing on holistic health outcomes, patient satisfaction, and effective resource utilization, demonstrating a commitment to the collaborative care model.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Successful completion of an approved Nurse Practitioner program. BSN required. MSN preferred.
- License/Certifications: Current nursing license issued by the State of Maine or eligible for licensure. Current BLS certification.
- Experience: Minimum three years of professional nursing experience. Two years of Nurse Practitioner experience preferred.
- Must be caring and empathetic and have great listening skills.
- Demonstrated record of consistently achieving clinical performance metrics.
- Knowledge of advanced practice of nursing, professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.
- Knowledge of patient assessment techniques including taking medical histories, performing physicals, evaluating health status, including state of wellness and compliance with care recommendations.
- Skill in case management, time management, problem solving, crisis intervention, multitasking and other organizational matters.
- Ability to make responsible decisions within scope of NP Practice.
- Ability to educate patients, families, and staff in a user-friendly manner.
- Strong computer skills with skill in using computerized health information management system.
- Excellent communication, interpersonal and customer service skills with the ability to establish/maintain effective working relationships with patients, clinical staff and the public.
- Conduct telehealth visits as part of our commitment to providing access to care. Must possess strong digital literacy skills, demonstrate proficiency with telehealth platforms, and maintain a high standard of patient care and communication in a virtual setting.
