AMHC is in search of a Nurse Care Manager (RN) in Washington County. This person is responsible for working collaboratively with a team focused on integrated, holistic care delivery, support, and prevention for individuals who have long-term psychiatric disabilities. The Nurse Care Manager will support a patient-centered, self-management approach to working with individuals to effectively assess their health care needs, identifying prevention, treatment goals, and management of their overall health condition. The chosen candidate will be expected to demonstrate the core values of AMHC by showing Integrity, Compassion, Excellence, Service, Collaboration, and Respect to co-workers, clients, and community members.
Qualifications:
- Licensed Registered Nurse
- Valid Driver's License
- Demonstrate AMHC's focus on Service and Excellence by having a strong understanding of the needs of the community and the eligibility for admission to programs.
- Build a Collaborative relationship with the healthcare teams and community supports.
- Show the Integrity of our programs by maintaining a comprehensive working knowledge of resources and target populations for service.
- Review client's medical needs with Respect and demonstrate the needed Compassion for our communities.
Experience Needed:
- Experience in nursing, public health, or case management with the coordination of inpatient, ambulatory, or community services.
- Experience working with individuals with chronic illness and providing care management, goal-setting, and constructive feedback.
- There is a requirement to participate in an on-call rotation with the other agency nurses for the Calais Children’s Residential Unit by phone.
