Family Medicine, Internal Medicine, and IM PEDS
SUMMARY
The purpose of this position is to provide high-quality patient care and ensure effective clinic operations. Patient care includes providing comprehensive medical services for family members, regardless of age or gender, within the scope of training. Services include but are not limited to: preventive care, immunizations, primary care and treatment, health education, prescribing medications, ordering and interpreting lab tests, physical examinations, STD screenings, vision/hearing, women’s health, family planning, prenatal services, and nutrition. The provider may work in various locations depending on the organization's needs. Clinic shifts include 1 evening a week and 1-2 Saturdays/month. This position may also include supervising physician assistants and/or nurse practitioners and providing training to resident physicians or other health provider interns/externs.
REQUIRED EDUCATION, EXPERIENCE, and COMPETENCIES
- Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care.
- Take patient histories.
- Perform or request diagnostic tests.
- Make diagnoses and prescribe medication as necessary.
- Administer vaccinations and perform follow-up examinations.
- Educate patients in disease prevention and health maintenance.
- Prepare all medical records, clinical reports, and correspondence.
- Support quality initiatives.
- Work in collaboration with the management team and providers.
- Complete all Core Curriculum training(s) required for the position.
- Perform all other duties as assigned.
Licensure/Certification:
- Active Illinois State Medical License, DEA certificate.
- Board Certified or Board Eligible based on governing board guidelines.
- Providers are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management.
Experience:
- Experience should be aligned within the scope of training.
- Familiarity with Electronic Health Records (EHR) is preferred.

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE
Graduate of an accredited medical school with a degree of Doctor of Medicine (MD) or Doctor of Osteopathy (DO); completion of Internship and Accredited Residency Program required.
OTHER QUALIFICATIONS
- Current driver's license and auto insurance.
- Current and unrestricted medical license in the state of Illinois.
- Current and unrestricted Drug Enforcement Administration Certificate.
- Complete all required Continuing Medical Education.
