About the Position:
We are seeking a LPN - Licensed Practical Nurse to work per diem to fill in open shifts. This is an excellent opportunity for those looking to maintain a great work/life balance.

Compensation:
- Hourly Wage Range: $32.00 to $37.40 per hour, depending on years of experience.
- Shift Differential: Additional pay on top of the hourly rate:
- Weekday 2nd and 3rd shift: +$2.00
- Weekend 1st shift: +$2.00
- Weekend 2nd shift: +$3.00
- Weekend 3rd shift: +$3.50
Per Diem Requirements:
Minimum Shifts: Work a minimum of 4 shifts a month, with at least 2 shifts on weekends (Saturday and/or Sunday).
Job Responsibilities:
- Comprehensive Care: Provide care to residents in accordance with CCNH policies, standards of care, and state and federal regulations.
- Care Coordination: Assess, plan, implement, and monitor resident care plans, collaborating with supervisors and attending physicians as needed.
- Supervision: Oversee and support LNAs and LMNAs in delivering quality care on the assigned unit.

Requirements:
- Graduate of an accredited school of nursing
- Long-term care experience preferred
- Must possess and maintain a current Practical Nursing license and be in good standing with the State of New Hampshire Nursing Board
