The primary purpose of this position is to provide for the day-to-day care needs of the residents in a Skilled Nursing Facility Environment. Provide supervision and leadership to the Certified Nursing Assistants in the delivery of resident care.
Job Responsibilities / Position Information:
- Provide hands-on care to the resident based on the resident Plan of Care.
- Administer medications and provide treatment to the resident based on the physician orders.
- Identify changes in the resident's condition and evaluate the resident care needs.
- Strong assessment and documentation skills.
- Evaluate and participate in the development of the Resident Plan of Care.
- Monitor the resident's overall condition and provide care as required while maintaining compliance with facility policy and procedures.
- Provide primary skin care to residents under the medical direction and supervision of the residents’ attending physicians or the Director of Nursing Services, with an emphasis on treatment and therapy of skin disorders.
- Identify, manage, and treat specific skin disorders, primary and secondary lesions, and decubitus ulcers.
- Ensure that residents with decubitus ulcers receive appropriate prophylaxis and treatment such as daily inspection, turning and activity, a well-planned diet, and maintaining a clean, dry bed.
- Must be able to push, pull, move, and/or lift a minimum of 25 pounds and/or carry such weight a minimum distance of 50 feet.
Requirements:
- Registered Nurse in good standing
- LVN in good standing
Knowledge / Experience:
- At least 1 year of Wound Care/Treatment experience
Licensure / Certifications:
- Current RN license
- Current LVN license
