OCES supports older adults and individuals with disabilities by providing information and coordination of services promoting healthy, safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities.
And we are growing! The Behavioral Health Team at OCES is seeking two Behavioral Health Clinicians to support our consumers in the community using a person-centered approach to address complex medical and behavioral health needs.
One of these roles must speak Haitian Creole to best serve the needs of that community.
What you'll do:
- Work directly with older adults with complex behavioral health conditions who require long- term case management and/or psychotherapy to remain safely in their community.
- Conduct home visits as needed to assess and develop comprehensive, person-centered care plans with consumers, caregivers, and families.
- Provide advocacy, support, and assistance to address unmet need including but not limited to housing, hoarding, substance abuse, medical case management, and communication with providers.
- Conduct outreach to local providers about Behavioral Health services.
- Coordinate and/or facilitate support groups based on community need.
What you'll bring:
- Master's degree in social work or related field; current LCSW/LICSW license
- Three years' clinical/community-based experience.
- Extensive knowledge of the psychological, emotional, and social needs of elders and people living with disabilities.
- Great assessment and communication skills needed.
- Strong ability to work independently.
- Bilingual highly preferred; Haitian Creole needed for at least one position
- Proficient with Microsoft 365 and other software.
