Job Summary
Manages and coordinates a specialty program or outpatient office. Provides clinical expertise, coordination of care, and education for patients, physicians, and hospital-based case managers in order to enhance and facilitate patient care. Manages assigned clinic personnel, assesses staffing needs, evaluates staff member performance, coordinates equipment maintenance, utilizes appropriate health care resources when needed, assesses staff member continuing education needs, delivers direct patient care, and observes departmental and hospital policies and procedures.
Minimum Requirements
Experience - 2 years clinical experience as a licensed nurse
Education - Associate's Degree in Nursing
Special Skills - Proficient in the English language, verbal and written communication skills, computer skills
Licensure - Current Louisiana RN license as applicable; BLS Certification
Responsibilities:
- Clinical Practice and Care Management:
- Performs psychosocial/physiologic assessments that integrate changing data.
- Oversees and performs selected nursing duties, including documenting patient history, vital signs, immunizations, and allergies. Prepares, stocks, and cleans exam rooms and maintains supplies and equipment for treatment.
- Adapts planned educational programs and information to individual patients and families by modifying teaching strategies or content. Integrates education during the delivery of patient care. Collaborates with patients/families to identify realistic desired outcomes.
- Ensures efficiency in the delivery of care, determines immediate priorities, and offers solutions for problems.
- Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict.
- Identifies variances from expected outcomes. Responds to patient's needs and provides holistic care using independent clinical judgment based on knowledge drawn from education and experience.
- Evaluates patient outcomes and makes revisions in the plan of care.
- Documents all patient care activities as per documentation standards. Assists the healthcare team with the documentation process.
- Oversees and/or takes responsibility for appointment and referral scheduling, chart management, and coordinating patient flow.
- Provides confidential analytical support for the Director of Operations on matters related to personnel including hiring, disciplinary action, and employee annual evaluations.
- Communication and Collaboration:
- Consistently communicates/collaborates with health care team members, patients, and families to maximize patient outcomes.
- Appropriately delegates patient care activities to other health care team members and participates in making and revising staff assignments to achieve maximum productivity.
- Maintains knowledge regarding unit initiatives and incorporates the outcomes of the team/committees work into practice.
- Participates on unit teams, work groups, and/or committees. Anticipates needs and voluntarily assists others.
- Manages the clinic and service line initiatives that are developed in collaboration with the providers to achieve goals related to internal & external relationships, growth in the community, and excellence in patient care.
- Acts as a liaison for the clinic and providers with team members who work on the inpatient side of other facilities within our health care system.
- Provides direction and guidance to others regarding practice and serves as a resource, preceptor, and mentor. Demonstrates leadership skills in critical thinking, decision-making, and problem-solving.
- Quality:
- Demonstrates knowledge of service line specific quality measures improvement initiatives.
- Participates in quality improvement initiatives by utilizing standards, guidelines, and pathways for care delivery.
- Incorporates evidence-based practice and research findings into nursing practice to enhance outcomes.
- Observes and adheres to all policies and procedures, and follows all safety, quality assurance, and infection control standards.
- Professional Development:
- Acts as a clinical resource and serves as a mentor to health care team members; displays leadership behaviors.
- Identifies own learning needs based on self-evaluation and the progress made from the previous year's goals.
- Accountable for all documentation and completion of all required continuing education and competencies relevant to the area of practice as defined by clinical management.
- Ensures improvements in practice settings by assuming responsibility for self-development in lifelong learning.
- Other Duties:
- Performs other duties as assigned or directed.
- Prepares performance appraisals, evaluates the performance of selected employees, and provides feedback to employees. Coordinates schedules and liaises with HR when employee issues arise.
- Provides leadership and demonstrates critical thinking ability in all areas of responsibility, treats all co-workers with respect, strives to enhance co-worker performance through cooperation and effective communication, and demonstrates hospitality and courtesy in all interactions.
