The role you’ll contribute:
The Pediatric Outpatient Diabetes Educator (RN), under general guidance, is responsible for providing age-specific and developmentally appropriate diabetes education to children, parents, and other healthcare professionals. The Pediatric Educator provides timely, effective, and appropriate diabetes education and clinical management support for patients with diabetes and their caregivers. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
- Provides pediatric diabetes instruction in group and individual sessions, in accordance with patient education standards set by the American Diabetes Association and departmental policy. Provides education in all content areas, except individualized meal plan calculation for RN, to all newly diagnosed children with diabetes and their families both referred from the inpatient hospital unit and those diagnosed and/or managed as outpatients.
- Obtains individual patient assessment. Identifies patient/family problems or learning barriers including physical, cognitive, environmental, language/cultural, financial, psychosocial, and spiritual and provides appropriate referral as necessary.
- Establishes individualized learning goals, objectives, and educational plan in collaboration with the patient and the patient’s caregiver(s).
- Evaluates patient upon completion of educational plan and provides remediation/plan modification as indicated and in consultation with diabetes team members as needed. Provides patient follow-up by telephone, mail, or e-mail as indicated and according to established department guidelines.
- Complies with the applicable standards for diabetes self-management education and self-management support set forth by the American Diabetes Association and with the Diabetes Education Policies and Procedures of the AdventHealth Pediatric Endocrine Clinic.
- Documents all educational assessment, planning, intervention, behavioral goal setting, and outcomes accordingly in the chart (electronic or paper as appropriate) accurately and within the expected time frame for charge entry, monthly statistics, and ADA recognition.
- Actively participates and accepts responsibility for special projects such as risk testing, speaker requests, support groups, and other community events designed to increase diabetes awareness. Participates in healthcare professional education programs as assigned.
Qualifications
The expertise and experiences you’ll need to succeed:
- Associate of Science Degree (ASN) in Nursing
- Current, valid State of Florida license as a registered professional nurse
- Advanced knowledge of pediatric diabetes concepts and skills including insulin pump therapy
- Ability to provide teaching in all aspects of diabetes management
- Ability to assess and interpret data relative to patient’s age and diagnosis
- Ability to work with children/families of various ages and various backgrounds-socioeconomic, cultural, etc.
Preferred qualifications:
- Bachelor of Science degree (BSN) in Nursing
- Certification with the National Certification Board of Diabetes Educators as Certified Diabetes Educator (CDE)
- One to three years of experience in Nursing
- One year of experience in diabetes specialty
