JOB LISTING: Hybrid Temp to Hire, Medicaid ACO Care Mgr. RN/LPN/LSW/LCSW Chicago, IL
Our client, an ACO located on the near west side of Chicago, is seeking an experienced Medicaid ACO Care Manager (RN/LPN/LSW/LCSW). The main purpose of this position is to provide a comprehensive assessment of patient risk and needs for education, discharge planning, and resource needs during an episode of illness and across the continuum of care. Experience with medical and behavioral health is required. This is a hybrid position; 3 days in office/2 days home)
Care Manager Job Responsibilities
- Creating and updating personalized care plans for patients.
- Collaborating with patients’ families/friends/social supports in developing treatment plans.
- Identifying and recruiting high-quality healthcare providers.
- Interacting with healthcare providers to facilitate patients’ treatments.
- Ensuring patients’ treatment requirements are met.
- Suggesting alternative treatment plans service requests do not meet medical necessity criteria.
- Educating patients on procedures, healthcare provider instructions, and referrals.
- Linking patients to social services programs i.e. transportation assistance and translation services.
- Conducting regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.
- Maintaining records of case management activities.
Care Manager Requirements/Knowledge & Skills
- Bachelor’s degree in social sciences, social work, nursing, or a related field.
- 2+ years’ work experience in public health or related field (ideally in a Medicaid ACO or FQHC)
- 1+ years current case management experience.
- RN, LPN, LSW, or LCSW: Certified Case Manager (CCM) license preferred.
- Some experience with mental health is a strong plus
- Highly proficient w/ Healthcare Management systems such as MHNConnect & MS Office; especially Excel.
- High degree of creativity/problem-solving ability & patience in navigating complex systems of care
- Ability to employ effective customer relationships with patients & healthcare team members
- Ability to work independently & also as a team with a wide variety of individuals
- Excellent oral, written, and interpersonal communication skills.
- Proficient computer skills & willingness to work some weekends
