UTHealth Houston Heights Clinic is hiring a Certified Diabetes Care and Education Specialist (CDCES) to join their team of professionals. The diabetes educator is fully integrated in a team based in a primary care and Endocrinology multicultural practice assisting with advanced diabetes care and education including pumps, technologies and basic diabetes education in individual and group sessions.
Preferences: Bilingual (English/Spanish), BC-ADM (Board Certified- Advanced Diabetes Management)
Of note: This is a credentialed position. Credentialing takes 4-6 months.
Schedule: Primarily Monday - Friday, 7:30am - 5:00pm / Minimal weekends and holidays

Position Summary:
Provides comprehensive Diabetes Self-Care Management Education (DSME) to patients and families as part of a collaborative multidisciplinary team in a clinical and administrative role. Responsible for maintaining expertise in education and diabetes self-management trends, technological devices and medications. This clinical role also serves to mentor clinic staff on downloading insulin pumps, meters, and continuous glucose monitors to enrich the patient experience specific to people living with diabetes.
Position Key Accountabilities:
- Provide diabetes education following American Diabetes Association and Association of Diabetes Care & Education Specialists guidelines via in-person appointments, incident to office visit appointments, virtual telehealth appointments, and diabetes support groups.
- Provide visual and hands-on education on continuous glucose monitors, insulin pumps, blood glucose monitors, and medications.
- Assess patient for medication adherence regimens and device options.
- Provide ongoing patient follow up phone calls for continued education and support.
- Collaborate with Providers, Community Health Workers, Social Workers, Clinical Pharmacists, Case Managers, and Health Educators to establish continuity of care via huddles and service requests.
- Maintain a patient schedule to comply with all regulations regarding billing for services. Serve as a team member to collect data or assist with other metric measures.
- Participate in health fairs and speaking engagements. Attend required staff trainings and meetings.
- Perform other duties as assigned.
Certification/Skills:
Certified Diabetes Care & Education Specialist
Registered Nurse or Registered Dietician
Bachelor’s Degree in Nursing, Dietetics Nutrition, or related field.
Three (3) years of related experience.
Physical Requirements:
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.

Residency Requirement:
Employees must permanently reside and work in the State of Texas.
