Job Description
As a Registered Nurse (RN) at Summit Health, you are a full integral partner in a multidisciplinary care team providing individualized team-based care. Under the direction of the Primary Care Provider, Clinical Practice Manager, and Office Supervisor, the RN works closely with the health care team to skillfully provide and facilitate the coordination of individualized total patient care. Through the application of the nursing process, you will accept accountability for the nursing care of assigned patients. This position provides leadership and clinical expertise in the assessing, screening, planning, evaluating, educating and delivery of care in accordance with professional nursing standards and patient needs. The RN will be motivated, energetic, and an empathetic communicator who is passionate about delivering the highest quality of care.
Essential Job Functions:
- Assist the physician and staff with the daily coordination of patient care and physician access.
- Function as the liaison between patients, staff, and physicians.
- Maintain efficient patient flow.
- Deliver direct patient care within the scope of licensure and in accordance with SMG policy.
- Collect lab specimens according to departmental guidelines. Accurately label specimens.
- Perform Point of Care testing, EKG’s and phlebotomy as applicable.
- Prioritize patient care by implementing physician orders and nursing procedures.
- Ensure proper preparation for patient visit. Prepare pertinent information needed for patient visit.
- Manage Nurse Visit schedule. Prepare pertinent information needed.
- Perform patient intake, check-in/check-out as needed.
- Effectively communicate problems, concerns or issues to the Office Supervisor/Manager appropriately and promptly.
- Assure all patients are correctly identified prior to providing and/or documenting care.
- Employ appropriate and timely use of Tasking in EHR.
- Manage Clinical Nurse/Assigned inbox.
- Obtain vital signs: BP, temp, pulse, height & weight accurately and completely as required. Document appropriately in EHR.
- Administer injections according to appropriate protocol. Document injections accurately in EHR. Administer VIS sheets.
- Perform patient outreach to satisfy quality measures (BP checks, vaccinations).
- Coordinate and facilitate daily huddles.
- Demonstrate appropriate telephone triage protocols and document in EHR.
- Schedule appointments appropriately and accurately according to department guidelines.
- Identify and schedule annual visits as applicable.
- Monitor daily hospital discharge list. Collaborate with Care Management for TCM appropriate visits.
- Adhere to universal safety precautions when administering medications and disposal of medical waste.
- Review and update medication list in EHR. Demonstrate understanding of prescription control and prescription refill procedures. Accurately and completely document known allergies in EHR.
- Initiate and follow through with prior authorization requests, DME orders, home health referrals.
- Enter orders and schedule appointments in the Practice Management System (PMS) and EHR.
- Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
- Check and maintain logs for emergency equipment according to policy.
- Facilitate transition of care to UCC/Hospital.
- Maintain proficiency with department specific equipment, troubleshoot and report malfunctions promptly.
- Actively participate in pilot programs.
General Job Functions:
- Assist in ordering and maintaining of all supplies as assigned.

- Exposure to infectious hazards, blood, body fluids, non-intact skin, or tissue specimens.
- Contact with patients or patient specimens are possible.
- Unplanned or unexpected exposure.
Education, Certification, Computer and Training Requirements:
- Associates degree required. Bachelor’s degree preferred.
- New Jersey State Nursing License and BLS certification required.
- 0 – 1 years experience. 2-4 years experience preferred.
- Ability to communicate in English, both orally and in writing.
- Strong interpersonal and organizational skills.
- Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
- Ability to use problem solving, critical thinking and priority setting skills.
- Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
- Experience with Standard Office Technology in a Window based environment preferred.
- Experience with patient care equipment preferred.
- Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.
Travel:
Ability to commute to satellite offices as needed.
